Sunset View Elementary School

The Boys & Girls Clubs of Utah County will be providing 7 days worth of shelf-stable lunch kits for children during Spring Break. Priority will be given to children attending schools where the Club is already serving meals, but other residents of Utah County are also welcome to apply. Parents will have the option to pick-up meals at our Boys & Girls Club Provo Clubhouse (1060 E 150 N, Provo, UT 84606) on April 2, 2022 or have meals delivered directly to their homes on April 4, 2022 (residents do not need to be home in order to receive the meals, as all components are shelf-stable).

Please complete this form by March 30, 2022.

Comidas durante las vacaciones de primavera

El Club Boys & Girls del condado de Utah proporcionará kits de almuerzo para los 7 días en los cuales los niños estarán en las vacaciones de primavera. Se dará prioridad a los niños quienes asisten a escuelas donde el Club ya está sirviendo comidas, pero otros residentes del condado de Utah también pueden aplicar. Los padres tendrán la opción de recoger las comidas en nuestra casa club de Boys & Girls Club Provo (1060 E 150 N, Provo, UT 84606) el 2 de abril de 2022 o recibir las comidas directamente en sus hogares el 4 de abril de 2022 (no es necesario estar en casa para recibir las comidas, ya que todos los componentes son estables).

Complete este formulario antes del 30 de marzo de 2022.
